After physician assistant Mia's change of heart in Paris (her boss is harvesting hearts from Chinese prisoners), she will be the heart of change in Beijing, where scientists are reengineering the genome of humans to treat diseases, and algae's DNA to treat pollution. Algae biofuels could be the low-emission fuel of the future. CRISPR, that cool genome editing tool, can also re-write geopolitics. Biology and I.T. have merged, but what will save Mia’s life in Xinjiang, a Muslim Chinese province? Yin, or Yang, or their reunion? Cutting or restriction enzymes in a democrature (mix of democracy and dictatorship) like China are not only cutting through DNA molecules. Lenoir, Mia’s ex-boss, is also in Beijing. After his bloody harvest in China, he will need to use CRISPR as a minimally invasive surgical tool to save the life of his ex-assistant. Maybe the perfect Yin Yang is when the irony of death is meeting with the irony of life?</div>

I used to be a teacher in middle and high school (foreign languages), and an executive assistant (or marketing), i'm almost fifty, and now working in real estate in Southern France (Pau). In my free time, i enjoy learning stuff, so i earned myself several MITx MOOCs certificates in <a href="http://web.mit.edu/mitxbio">biology and genomics</a>. Also, i love writing fiction. Nobody can understand why i write science-fiction (my culture in sci-fi is almost non-existant), but i just finished writing a techno-medical thriller, action taking place in China ("Airpocalypse") and would like to write "made-in-China" sci-fi. My co-writer Isa and myself are trying to finish another sci-fi, Jesus CRISPR Superstar, French title: La Passion du CRISPR.<br />

I have a great family, scattered around the world (France, NY, Malaysia), spending a lot of time interacting with those folks.<br />

My husband keeps motivating me, he is my learn-new-things coach, so i'm now getting ready for third part of MITx Molecular Biology MOOC: Translation. After that, i'd like to learn more about bioinformatics and precision medicine...<br />

I live in Pau (Southern France) and since i love Asia, getting ready for a 3-months-stay in Japan (spent a lot of time in China last year)... Since i'm interested in science people, twitter is a great way to build a virtual network allowing me to get updates about stuff i enjoy learning and writing about. When i was a teenager, i wanted to become a dancer, a choreographer, a surgeon or a writer. Nine fictional pieces in progress; some of them i write alone, some with the help of a co-writer...<br />

J'ai eu l'occasion de croiser Marie Berry à plusieurs reprises, elle m'a toujours impressionnée. Dans le bon sens. Tandis que les <i>conquistadors</i> de France Adot, héros malgré eux de la "wacky science" ... euh, c'est quoi, la mort encéphalique ? Je vous offre quelques cours sur ce sujet, faits par la Harvard Medical School ? La science de la mort encéphalique est tout sauf bordée, dommage, tout le don d'organes dit "post mortem" repose dessus ... Les toubibs institutionnels n'aiment pas trop qu'on les chatouille sur ces sujets. Et Big Pharma paie pour les campagnes d'"information" sur le don d'organes (les vendeurs d'immunosuppresseurs) ... <br />

Bref, le monde tournait un peu en rond tout en faisant son beurre, comme d'habitude, avec quelques zélotes du "Don" ... Science et religion mis en baratte, t'inquiète pas, j'fais mon beurre ...<br />

Et puis, mort du pionnier de la greffe cardiaque Christian Cabrol, dont les "insiders" savent qu'il n'était pas très favorable au don d'organe de son vivant. Or le rein est l'organe le plus demandé au monde. Il peut se donner de son vivant, la preuve ... Pour le vieux Cabrol (grand homme par ailleurs), point de salut hors du don d'organes dit "post-mortem" (euh, les organes d'un mort ne sauvent personne, mais ceux d'un mourant, cela se pourrait) ... vous voyez un peu la ligne grise qui se profile à l'horizon ? Cabrol (RIP) et consorts n'aimaient point que l'on parle de cette ligne grise, pourtant évoquée dans les cénacles de chirurgiens transplanteurs débattant entre eux ... Eh oui, ce modeste blog n'a rien inventé, il ne fait que recycler ...<br />

Bravo le frère et la soeur, beau tandem tout en finesse, vous redonnez de l'espoir et du sens à ce genre d'initiative : la campagne pour le don d'organes. Ce qui, pour moi qui ai démarré ce modeste blog en 2005 afin de donner l'alerte sur les campagnes d'"information" biaisées, n'est pas un vain mot ...<br />

The reason i met with this famous transplant patient, a cancer patient, had nothing to do with mHealth hype. It had to do with intelligence regarding brain death science and organ trafficking in China. It encompassed geopolitical stuff, surgical stuff, and human rights. We are talking about China, aren't we, dear jacket? Not wanting to be manipulated, i (wearing you) went to see my former professor of Geopolitics, <a href="https://medium.com/@cathcoste/the-impatient-patient-is-geopolitics-and-it-will-see-you-now-ebf9d8974b90"><span class="s2">Jean-Christophe Victor</span></a>. Discussed with him what i had told the patient who is painted on your back, dear jacket, seeking advice. Wanting to stay under the radar, i thought studying biology and genomics with you and earning ourselves MITx MOOC certificates would compensate for Regina's painting efforts and generosity. And you too, dear jacket, thought this was a great idea. Oh, we got on so well together, didn't we? We had so much fun learning about digitised biology. It wasn't always easy, though. There were days i wasn't quite sure if you were a congress uniform (business smart dress code) or a zip-up sweatshirt.</div>

Turns out me and my jacket, we didn't get "touched" by <a href="https://www.inverse.com/article/32209-air-pollution-smog-beijing-china-cigarettes-unhealthy"><span class="s3">airpocalypse</span></a> in Beijing a couple of years ago; i'm afraid it's the opposite. Both of us could "touch" that thick, polluted air with our fingers instead, when travelling to Beijing (that human rights activist thing, remember?). </div>

The whole thing must have been brewing inside me, a modest blogger and MOOC student, for some time, though, I guess. Flashback of people being slaughtered for their organs (yes, i had witnessed such things in China, having worked for surgeons), air pollution killing people, the arrogance of power (some establishment doctors: transplant surgeons, and other VIPs) showing too little concern about human rights in China (great source of transplantable organs, worldwide)... Both of us, as you know only too well, dear jacket, started writing (we had just completed MITx MOOC 7.28.x part 2 and we were exhausted, but never mind that) and refused to move from that Beijing airport (leaving China) without first talking to Beijing authorities. It was in winter 2015, do you remember? Neither of us was fluent in Mandarin Chinese, far from that, but after three or four days spent at Beijing airport (which we now know like the back of our hands: showers, restrooms, water dispensers for Instant Noodles, etc), we finally managed to speak to someone. In a mix of broken Mandarin Chinese and English, we both did our best to explain we were MITx students and had things to tell regarding geopolitics in China. All you remember from this period is me as a nervous wreck, refusing to admit i would be blocked from going to China because i had worked with Canadian human right activists who had accused the government of crime against humanity (human organ trafficking with the complicity of Chinese army). At this point i must thank you for your help. You successfully conveyed the message to Beijing authorities that we were both scientists (MITx students) willing to help Chinese people get excited about science, synthetic biology, CRISPR, genomic precision medicine and the potential of this new science to battle air pollution and cancer. I (fashion-) showed you to Chinese politicians, and little by little the craziness of the situation settled down. Police told us they wanted to hear what we had to say because they thought we both were very sick (true, duh, that pollution thing). The incident was "settled" (no money involved, only good will from all parties), and somehow (i will never pretend i understand what is going on in China and how networking kind of works there), we managed to build a small network, explaining we wanted to write "made-in-China" science-fiction. Even more determined than when learning for those MITx MOOCs, we settled to work and imagined the story we would both write. Yup. My jacket is my dear co-writer. Thank you for that, jacket. You were a great co-writer. We have come up with a 300-page-fiction (PDF), getting ready to be published as a book (French, Chinese, English). Congratulations, trilingual jacket! You made it ;-) What helped us through the ordeal is our Chinese network (businessmen, students, scientists from academia, all of them great fan of yours, dear jacket), and our passion for made-in-China science-fiction. </div>

When you find yourself not being able to breathe, and see others suffer around you, you come to think that there must be more to a bloody congress uniform painted by Regina than just hype, mHealth yada yada yada. Wouldn't you agree, dear jacket?</div>

I am fully aware, dear Jacket, that writing this techno-medical thriller, "Airpocalypse", is certainly not the task that was assigned to us by Regina. <span style="font-size: small;">Il faut savoir qu'une infection pulmonaire résistante peut tuer en quelques jours, surtout une personne âgée fragile (82 ans) qui se réveille d'un coma de dix jours suite à complications post-opératoire (opération de la hanche, au réveil, on découvre un colon nécrosé), et à qui on a enlevé le colon et une partie de l'intestin grêle, plus deux mini AVC dans un passé récent, qui font qu'il est sous anti-coagulants. </span><span style="font-size: small;"><br /></span><span style="font-size: small;"><span style="background-color: white; color: #222222; font-family: "arial" , sans-serif;">En cas de problème d'infection pulmonaire, il y a un protocole récent basé sur la génomique qui consiste à envoyer à un labo (Pasteur, Lyon, Rennes, Lille) des échantillons pour qu'ils analysent l'ADN des bactéries pour déterminer à quoi elles sont résistantes ou sensibles. Est-ce que vous savez si cela a été fait par l'Hôpital Européen Georges Pompidou ? (car eux n'ont probablement pas le laboratoire pour le faire).</span></span><span style="font-size: small;"><br /></span><span style="font-size: small;"><span style="background-color: white; color: #222222; font-family: "arial" , sans-serif;"><b>L'examen en question s'appelle : le microbiome pulmonaire</b>, qui aurait révélé quelles bactéries étaient présentes dans ses poumons, et quels gènes de résistance, pour un meilleur choix de l'antibiotique.</span></span><span style="font-size: small;"><br /></span><span style="font-size: small;">Le test de microbiome est un test de séquençage ADN (génétique) de l’ensemble des bestioles qui vivent dans notre estomac ou bien dans nos poumons (ou encore tractus génital, surface de la peau, bouche oreilles…etc).</span><span style="font-size: small;">Ca sort des séquences ADN en paquet, et là-dedans, on est capable de retrouver celles qui appartiennent aux bactéries, et lesquelles, celles des champignons, et celles des virus.</span><span style="font-size: small;"><br /></span><span style="font-size: small;">Quand on identifie comme ça les bactéries, on peut ensuite, dans un 2ieme round "bioinformatique", rechercher les gènes de résistance aux antibiotiques connus chez ces bactéries. Oui ça existe les bactéries résistantes, elles s’adaptent. Et donc, choisir en fonction, le bon antibiotique.</span><span style="font-size: small;">A noter que ce test de microbiome identifie des bactéries qui sont difficiles à cultiver, et que la microbiologie traditionnelle ne peut pas identifier.</span><span style="font-size: small;"><br /></span><span style="font-size: small;">Donc (ma blague à deux balles) dans les cas d’infections graves, un bon microbiome vaut mieux qu’une mauvaise boite de Petri.</span></blockquote>

<span style="font-size: small;">A resistant lung infection <a href="http://www.fungihealth.com/maitake-medical-mushroom-extract-vs-pneumonia-and-lung-infection">can kill within a few days</a>, especially a frail elderly person (82 yo) waking up from a coma due to postoperative complications (colon and part of the small intestine were removed, <span style="font-family: inherit;">suffered two strokes in the recent past, and as a consequence, taking anticoagulant or blood-thinner treatment). Before his coma, he was in a clinic in Paris (hip surgery). Then the complications happened (necrosis of colon)... </span><span style="font-family: inherit;">For lung infection, there is a recent protocol based on genomics which consists in sending samples to a laboratory (Pasteur, Lyon, Rennes, Lille) to analyze the DNA of Bacteria to determine what they are resistant to or sensitive to. Do you know if this was done by the European Hospital Georges Pompidou? (Because they probably do not have the lab to do it). </span><span style="font-family: inherit;">This examination is called: the pulmonary microbiome, which would have revealed which bacteria were present in the lungs, and which genes of resistance, for a better choice of antibiotic. </span><span style="font-family: inherit;">The microbiome test is a DNA (genetic) sequencing test of all the bugs that live in our stomach or in our lungs (or genital tract, skin surface, mouth, ears ... etc). When we identify bacteria in this way, we can then, in a second "bioinformatic" round, look for the antibiotic resistance genes known in these bacteria. Yes there are resistant bacteria, they adapt. And so, choose the right antibiotic accordingly. With this big-data DNA (lots of DNA sequencing data), what can be done? This microbiome test identifies bacteria that are difficult to grow, and that traditional microbiology <i>cannot</i> identify. In cases of resistant (lung) infection, seek help from the microbiome, not from the petri dish!</span></span><br />

<b><i><span style="color: blue;">A creative person, with a love for interprets, interested in science people.</span></i></b> I tried writing a couple of fictions over the last couple of years (as a full-time job); now working with a movie production company in Taiwan. Writing is like everything else, i guess: hard work. It keeps me motivated to think about interprets i would love to see acting or singing in the various roles in the medical thriller and the sci-fi i'm writing. My co-writer Isa, who has been working with me for years, is a key person in this writing process. She's like some kind of doppelgänger i would guess... She is teaching international cooperation in various universities and has been travelling around the world. I suspect she is the reloaded version of French writer (and opera singer) Alexandra David-Neel, the first woman to travel to Tibet and India and to write about it. She is an amazing storyteller, never fearing to experience what it's like to be a woman in Bangladesh, in rural places in India, or diving into an unknown ocean at the other end of the world for a couple of hours just minutes after she was taught how to dive in the first place.<br />
My (much less adventurous) background includes ballet, singing, a PhD in (German) literature, as well as a constant effort to seek updates in life sciences (genomics and medicine mainly). That's how i earned myself four MITx MOOCs certificates in biology.<br />
Fluent in a few languages: English, German, French, also Lithuanian (!!) and Dutch. Passion for Mandarin Chinese language; trying to learn how to speak and write Chinese in my free time. Eager to practice, I speak to my husband who cannot even say hello in Mandarin Chinese (but knows his way around each and every computer coding language!), or to my stir-fry pan when standing in the kitchen and getting dinner ready, including the trees and ducks when i go for a walk in the park. In my family, everyone has lived and worked in various countries and is speaking at least three languages (my brother, an engineer, speaks 8!) Interestingly, my mom taught Spanish to my brother while i was being taught German from another part of the family. My granddad taught me English (through literature), which my mom only speaks when forced to. My dad (you wouldn't guess) is a rebel (fluent in Italian, but hardly anybody else in the family is. Ha!) We've got interesting family gatherings in various countries (even continents) where a bit of everything is spoken. My brother's in-laws mainly speak Cantonese. A true melting pot. <br />
<br />
When i was a student, with (already) a passion for books, Elias Canetti (also from a polyglot family) captured my attention. There will be no need to learn languages now, as deep learning will teach this skill to machines, Google translate and stuff. Still... Connecting with people in their native language (or in any language they are truly mastering) seems as important to me as the ability to feel, to touch, to smell, to hear... Currently studying (as an autodidact) Chinese authors of sci-fi. Their work, their bio. <br />

Aubrey de Grey, scientifique spécialiste du vieillissement sans doute le plus connu au monde, dit que le fait de vieillir, appelé "aging" en anglais, est aussi une maladie. Je dirais donc qu'il y a des silos (ou réservoirs de matière agricole) avec plein de maladies, et que jusque là on a toujours cherché à traiter silo par silo ... C'est la médecine conventionnelle. Ne pas mélanger les torchons et les serviettes. Silo cancer, silo sclérose en plaque, silo Muco, etc .</div>

Car à bien y réfléchir, cette recherche médicale qui tâtonne entre des maladies en silos est absurde ...</div>

J'ai promis de résumer la matière scientifique décrite dans ce livre, ce que je tente ici. Merci aux Editions Stock de ne pas m'en vouloir pour les nombreuses citations, éclairantes pour comprendre ce qu'est la biopsie liquide et son rôle dans le dépistage et traitement précoce du cancer. J'espère que ce résumé plus approfondi incitera davantage de public à lire ce livre remarquable ...<br />

<b>Le principe de la biopsie liquide :</b><br />
<br />
Comment prendre en filature les cellules tumorales circulant dans le sang bien avant que la maladie ne se signale sans se tromper de cible ? Tout repose sur un terme qui n'est familier à personne en Occident : le diagnostic précoce. La plupart du temps, les gens ont un cancer stade 2 ou 3 ou même 4 ... Avez-vous jamais entendu parler de patient dépisté à un stade zéro ou un ?! ... Ce serait marcher sur la tête ? Si on arrive à relever le défi (ne pas se tromper : dépister les vrais ennemis, et non créer de faux ennemis), voilà qui serait une véritable révolution. A ceci près : depuis toujours, la médecine chinoise repose sur la prévention ... Pour eux, c'est nous qui marchons sur la tête ...<br />
<br />
Une méthode basée sur une prise de sang permettant de détecter le cancer avant l'imagerie ? Voilà ce dont il est question ici. Tout tourne autour de la mise au point d'un test. ISET.<br />
<br />
"Octobre 2014. Après presque 20 années d'efforts de la part de toute l'équipe de recherche que je dirige, j'apprends que nous avons gagné une bataille décisive. Le test sanguin que nous avons mis au point et qui consiste à filtrer le sang pour y trouver d'éventuelles cellules cancéreuses circulantes (CCC2) , dites "sentinelles", lesquelles signalent la présence du tueur qui rôde dans le corps humain avant même qu'il ait commis l'irréparable, ce test, donc, a fonctionné !"<br />
<br />
Le test de dépistage sanguin peut constituer une alerte plusieurs années avant que le cancer ne soit visible sur une radiographie ou un scanner ! Une photocopieuse qui accomplit une tâche d'isolement, associée à l'analyse experte des cellules au microscope ... Le tout, proposé demain, pour le prix d'une simple numération sanguine, à tout un chacun ? ISET est en fait une machine à filtrer le sang prélevé sur un patient (prise de sang). Cela permet de "repérer si un ennemi se prépare à l'attaque dans chaque organisme, et de le mettre hors d'état de nuire."<br />
<br />
"Le Test ISET permet d'intervenir très en amont, alors que 75% des cancers du poumon ne peuvent plus être opérés quand ils sont détectés, parce qu'ils en sont à un stade trop avancé et ont déjà produit des métastases."<br />
<br />
<b>Le danger du faux-positif :</b><br />
<b><br /></b>
C'est la fausse alerte, alors que tout va bien. Il faut savoir que suite à la compression d'un organe à cause d'une chute ou d'un choc, des cellules d'organe parfaitement normales peuvent se retrouver dans le sang. Nos organes sont comme des éponges et il ne faut pas grand-chose pour qu'ils diffusent dans le sang un très petit nombre de cellules." Conclusion : les cellules d'organe présentes dans le sang ne sont pas forcément tumorales ... Gare aux faux-positifs ! Apparemment, des concurrents du test ISET sont tombés dans le piège ... Autre hypothèse qui s'est avérée erronée : si on part du principe (faux !) que les cellules d'organe présentes dans le sang sont forcément tumorales, on va développer "un système et une machine pour détecter les cellules d'organe dans le sang, pensant (à tort) que toutes les cellules d'organe, y compris les cellules tumorales dérivées d'organes, fabriquent des protéines dites "épithéliales" (qui se situent dans leur membrane)" ; pour cela on va fabriquer des anticorps, c'est-à-dire d'autres protéines qui les reconnaissent et se lient à elles. "En mettant le sang en contact avec les anticorps puis avec un aimant (attacher les anticorps à des molécules magnétiques au préalable), les cellules d'organes qui se sont collées aux anticorps peuvent être extraites du sang". Or il s'avère que lesdites "protéines épithéliales sont absentes dans un pourcentage variable de cellules tumorales, le plus souvent la moitié d'entre elles environ."<br />
<br />
C'est au MIT, 10 ans plus tard, qu'en a été trouvée la raison. Le mécanisme responsable de la perte des protéines épithéliales dans certaines cellules tumorales. Voilà qui invalide la stratégie basée sur des anticorps (les concurrents du test ISET) ... En l'occurence, ici on produit de faux-négatifs (la non-détection d'une maladie), puisque la méthode ne parvient pas à capturer toutes les CTC présentes dans le sang.<br />

The clinical applications of CTCs include the prediction of cancer prognosis; selection and monitoring of therapeutic regimens; and drug target applications. On voit que l'équipe de la chercheuse a dû naviguer (avec succès) entre deux écueils : celui des biopsies liquides trouvant de faux positifs et celui inverse (les faux négatifs) ...<br />
<br />
C'est en 1995 que l'on trouve que le processus d'invasion tumorale, dans le cas du cancer du sein, débute environ 5 à 10 ans avant le moment où le cancer est diagnostiqué par des méthodes classiques d'imagerie comme la mammographie. De là, Patrizia mettra tous ses efforts pour développer un test sanguin prédictif. Elle entrevoit cette possibilité en 95 ... Le livre est loin d'être un compte-rendu de matière scientifique desséchée ... On voit que tout a été ressenti, testé, éprouvé, à l'aune de la maladie et de la perte d'êtres chers et de patients ... La science navigue entre ces tranches de vécu, en cela je dirais que Patrizia est le nouveau David Khayat, qui lui aussi a, en son temps, raconté les avancées scientifiques de la cancérologie (et ses échecs) à travers le vécu de tant de patients ... dont son chef de service, mort jeune. En quoi les CTC se distinguent des cellules du sang ? Voilà la question qui va travailler la chercheuse jour et nuit durant presque deux décennies ...<br />

D'une nature profondément humaine et désemparée face à la douleur de ses malades, leurs interrogations, leur demande d'aide, ne supportant plus de voir le corps médical adopter des protocoles ridicules et inefficaces, pour des malades condamnés, elle a depuis vingt ans étudié cet ennemi qu'elle assimile à un ivrogne d'abord peu agressif au début, mais subitement dangereux qui au bout de quelques temps se transforme en une bande d'individus dangereux ! C'est donc une sorte de course poursuite à laquelle elle se confronte, en voulant prendre le cancer de vitesse. Pour elle, c'est un serial Killer dont l'arme principale est le temps. Deux ennemis à prendre à revers : le tueur et le temps qui est son couteau meurtrier.
On lira tout son parcours et son affliction face à la détresse de ses malades, en début de son ouvrage, écrit remarquablement, au travers des lignes on retrouve toujours certains de nos proches qui sont passés par ces terribles épreuves. Nous aussi devant leurs souffrances, nous étions malheureux et impuissants, ne sachant à quel saint ou sainte nous vouer...
Grâce à son test Iset capable de repérer les signes avant-coureurs de la maladie par une simple prise de sang, Patriza est en passe d'arriver au but qu'elle s'est forgé,
Avec un prise de sang, les cellules circulantes, issues du cancer primaire sont tout de suite repérées lors de l'analyse, avant de se transformer en métastases en colonisant d'autres organes sains. On peut demander à ce point d'investigations des examens divers tels que mammographies ou scanners.
Ainsi à titre d'exemple :
l'équipe du professeur Paul Hofman, du CHU de Nice, a suivi pendant six ans une cohorte de patients à risques – des gros fumeurs atteints de bronchopathie pulmonaire. Grâce au test, les chercheurs ont détecté des cellules tumorales dans le sang de cinq d'entre eux bien avant que le cancer du poumon soit visible par radiologie. Ces patients ont été opérés et guéris. Une preuve de l'efficacité du test.
Actuellement le test n'est pas remboursé par la sécurité sociale et coûte 386 €.
Il faut encore attendre, nous conte Patricia Brechot, que toutes les formalités soient remplies. Elle se heurte aussi aux différents concurrents pharmaceutiques et laboratoires, bien que 50 brevets prouvant l'efficacité de ce test aient été déet validés. On comprends que certains n'apprécient pas ce genre de découverte révolutionnaire et salvatrice.
On ne peut qu'encourager et féliciter Patrizia Paterlini pour son acharnement, sa patience, son intelligence et sa grande bonté envers le genre humain.
Un livre qu'il faut acheter ét promouvoir pour arriver à obtenir un remboursement pour ce test, il est vrai assez onéreux pour l'instant mais sans doute miraculeux pour tout un chacun de nous." (Source : Amazon)<br />

J’étudie depuis trois ans la biologie moléculaire et la bioinformatique grâce à des cours en ligne gratuits (MOOCs) du MIT ou Massachusetts Institute of Technology. Après mes quatre premiers diplômes (je me prépare actuellement au cinquième) et me trouvant en France pour un bref passage, je tombe sur ce livre, un peu par hasard. « Tuer le cancer », par le Professeur Patrizia Paterlini-Bréchot. Prix Essai France Télévision 2017, Stock Editions.<br />

Les biopsies liquides, ou la thérapie du futur pour la médecine génomique de précision, j’en entends parler en long, en large et en travers depuis les livres du Dr Eric Topol, célèbre médecin généticien américain, ou des articles de l’excellente revue technologique du MIT (MIT Technology Review). Le citoyen français n’a pas le droit de faire séquencer son génome de son propre chef. Il s’expose à une amende et à de la prison, dixit les lois bioéthiques. Bigre … Début mai 2017, une société génétique américaine de grande qualité (spin-off de Harvard) a passé des accords avec un vendeur de tests génétiques en Espagne, Be My Gene. On voit bien l’obsolescence de ladite interdiction gauloise.

Pourquoi mon coup de foudre pour ce livre, unique en son genre en France, pays très en retard dans les domaines de séquençage du génome ?<br />

1) D’abord, pour son honnêteté. On est très loin de la frime malhonnête que le titre pourrait laisser craindre. Transformer la moitié des cancers d’ici 30 ans en maladie longue durée, voilà ce qu’on peut espérer. Tuer le cancer demain ? Attention, arnaque. Or Patrizia est une guerrière, et elle expose des difficultés et obstacles (dont elle a triomphé) qui feraient baisser les bras à la plupart d’entre nous.
« Grâce à son test Iset capable de repérer les signes avant-coureurs de la maladie par une simple prise de sang, Patriza est en passe d'arriver au but qu'elle s'est forgé, Avec un prise de sang, les cellules circulantes, issues du cancer primaire sont tout de suite repérées lors de l'analyse, avant de se transformer en métastases en colonisant d'autres organes sains. On peut demander à ce point d'investigations des examens divers tels que mammographies ou scanners. Ainsi à titre d'exemple : l'équipe du professeur Paul Hofman, du CHU de Nice, a suivi pendant six ans une cohorte de patients à risques – des gros fumeurs atteints de bronchopathie pulmonaire. Grâce au test, les chercheurs ont détecté des cellules tumorales dans le sang de cinq d'entre eux bien avant que le cancer du poumon soit visible par radiologie. Ces patients ont été opérés et guéris. Une preuve de l'efficacité du test. Actuellement le test n'est pas remboursé par la sécurité sociale et coûte 386 €. Il faut encore attendre, nous conte Patrizia Brechot, que toutes les formalités soient remplies. Elle se heurte aussi aux différents concurrents pharmaceutiques et laboratoires, bien que 50 brevets prouvant l'efficacité de ce test aient été validés. On comprend que certains n'apprécient pas ce genre de découverte révolutionnaire et salvatrice. On ne peut qu'encourager et féliciter Patrizia Paterlini pour son acharnement, sa patience, son intelligence et sa grande bonté envers le genre humain. Un livre qu'il faut acheter et promouvoir pour arriver à obtenir un remboursement pour ce test, il est vrai assez onéreux pour l'instant mais sans doute miraculeux pour tout un chacun de nous. » (Source : Amazon)

"Ce livre donne de l'espoir aux malades et avise ceux qui courent le risque d'être atteints . La passion , l'acharnement de ce médecin et de ses équipes , avec des moyens de plus en plus sophistiqués et un brin d'empirisme ont abouti à la détection des cellules cancéreuses par une simple analyse de sang ! C'est formidable , extraordinaire ! Chapeau , c'est écrit avec humanité , simplicité , émotion et chaleur ! Bravo Madame !
Mais cet examen dérange , la prévention dérange !
Il y a une économie que vous risquez de bouleverser !
Plus de prévention , ce sont moins d'examens , moins d'hospitalisations , moins de médicaments .
Le malade risque de passer après les retours sur investissements des professionnels de la santé !
Alors oui , votre découverte et extraordinaire , l'imposer sera une autre paire de manches !

Donc on apprend qu’un test a été mis au point. ISET. Saluons ce résultat tangible, là où tant de recherches n’ont pas abouti. Les risques du métier …
Ce qui m’a le plus attristée dans ce livre : Aussi triste que la maladie elle-même: l'hyper compétitivité, les guerres des silos/tranchées entre scientifiques - chercheurs - médecins. Et c’est pareil dans tous les pays, car c’est le système qui est construit ainsi … Attendons de voir comment l’Asie va bouleverser le domaine … car pour le moment il semblerait qu’ils soient fort bien placés pour prendre le leadership mondial en matière de médecine de précision (Precision Medicine) … La classe moyenne la plus importante en nombre se situe actuellement en Asie, ce qui place d’office le continent comme moteur pour les avancées technologiques à venir (dont la médecine) … En Europe et aux USA, la classe moyenne souffre et se trouve comprimée.<br />

2) Ensuite, pour sa pédagogie. Ce qui est expliqué ici, c’est la démarche scientifique ayant permis de mettre au point un test fonctionnel (et non une machine à faux positifs). Un faux positif est un diagnostic de maladie quand en fait le patient va bien. Le propos est remarquablement bien mené. Quiconque veut comprendre les enjeux de la médecine de demain se doit de lire ce livre. Un guide précieux pour comprendre la révolution qui se prépare et ne se fera pas sans nous, les patients. En résumé, il y a quatre silos, ou tranchées ne communiquant pas entre eux ou elles. Le dossier médical personnalisé du patient (pour le moment, un mirage), le génotype (les données génétiques du patient), le phénotype (données observables et quantifiables, non génétiques : comportement, style de vie, éventuelle maladie), les objets connectés (IoT). Le cinquième élément, dirait Luc Besson, est le patient.<br />

3) Enfin un test qui permet d’espérer des avancées françaises. Sans quoi, les Français seront condamnés à choisir entre la e-santé américaine ou chinoise … Pour finir, je me permettrais un avis personnel. Soigner le cancer avant qu'il n'arrive grâce à la biopsie liquide. Un test a été mis au point. Aura-t-il la peau des silos ? On nous parle à longueur de temps de deep learning dans le médical. Mais n’oublions pas que pour nourrir ce mystérieux procédé A.I., il faut des données qualitatives en très grand nombre. Les silos ne permettent pas ce partage. A trop parler de deep learning en nous faisant oublier l’importance d’être, nous, le patient, propriétaire de nos données, on risque de mettre la charrue avant les boeufs. Le partage massif de données reste un pré-requis du deep learning dont la loi est « garbage in, garbage out. » En remettant le patient au centre du système de santé, Patrizia se positionne comme un K.O.L. (key opinion leader) français de la e-santé, et on en avait bien besoin. Aux USA ils ont un Dr. Eric Topol, en Europe il y a le Dr. Berci Mesko, et maintenant en France on a, rien que pour nous, Madame Biopsie Liquide.<br />

Une dernière chose : que feriez-vous si vous pouviez avoir vos données génétiques sur votre smart phone, les gérer dans un cloud privé et les partager avec qui vous souhaitez, en toute confidentialité ? Permettez-moi de vous présenter Portable Genomics, startup française ayant le potentiel d’un exploseur de silos. Mariez cette startup visant à démocratiser les données médicales (le Thomas Piketty de la e-santé) avec Madame Biopsie Liquide, et vous aurez enfin une alternative française qui tient la route. Sans quoi, il ne restera au patient français de demain qu’à se tourner vers les USA ou la Chine pour gérer sa santé, pour le meilleur comme pour le pire …<br />
